Implement from name and from address overrides
Description
Implement parameters to override the from name and from address so it sends from the site instead of sending from the users email address. Implement reply to name and reply to address as well. By default it'll be blank, but if you specify a form name and from address it'll default to the users name and email address. Both of these usages should support substitutions.
