Redesign media displays
Description
Redo the media outputs to be HTML. Additionally implement a modal window usage like new CB Gallery. Photos should just simply have the photo inline. Clicking it should open the modal window (modal window should have commenting that's just the commenting on the activity entry). Video and Music should just be an inline player (no modal). Files should be a file details table with download button (in short support those 4 media types keeping in line with CB Gallery). URLs should be boxed with photo, title below (linked), description, then source (just domain).

The modal window for activity media doesn't need any additional functionality beyond providing the modal window. So it should be simple like CB GroupJive.

File attachment handling is implemented. Forget adding a model window. It's just an unnecessary addition.
