Comment activity causing duplicate comment output
Description
If activity from a comment and the activity that's commented on is on the same page the comment is duplicated. The menu for the original activity entry is also being suppressed when that's only supposed to be for the embedded activity entry.

The limitstart is being reset to 0 causing the paging to output when it shouldn't.

It's due to the query being cached so limitstart is never updated with the count. This won't be an issue or needed anymore when paging is changed to more accurate timestamp based.
