Improve activity and comment notification title handling

Description
Currently html is tripped and the title is then linked to the activity entry. This is fine for a normal post that actually contains a message, but for comment notifications on activity entries that only contain a CB Gallery upload for example this causes a pretty odd title. This behavior should be removed for plugin activity title parsing. It was originally needed due to being the only way to link to something, but with #7209 it's no longer necessary since the entire container can link to the entry.

The title replacing still needs to happen otherwise it becomes too confusing as to what the notification is on. In the above example it makes it seam like the notification is on the photo, but it's not. It's on the activity post.
