Re-check commented on and replied to notifications to ensure they properly link to comments.

Commented on notifications should link to original sources (e.g. for CB Gallery open the modal window).

Replied to comment notifications should link to the comment replied to.

Confirmed "commented on your CB_GALLERY_MEDIA" notifications are not opening modal windows on click. Direct link does work at least. Still need confirmation on comment reply notifications.
