Missing category and group approval notifications

Description
When a user creates a group while group approval is enabled and a moderator set themselves to receive an approval notification then one is never sent. Perhaps it is, but is being sent as PM, but expecting email?
There is no group approval notification, needs to be added. User approval notification is working fine.
- Status changed from Assigned to Resolved
- % Done changed from 0 to 100
- Subject changed from Group approval notifications not received - UNCONFIRMED to Missing category and group approval notifications
- Status changed from Resolved to Closed
