Remove dependency on formatname field

Description
Currently name display is dependent on the formatname field, which users tend to hide from profile, unpublish, or move to unreachable tab. Change name construction to use direct object variables and the name format function to prevent further name display issues.

Few minor locations still missing the upgrade. Need to review doing same for online status and for avatar to avoid dependency on the field needing to meet specific conditions.

- % Done changed from 80 to 100
Upgraded all locations to use the full access parameter.
- Status changed from Resolved to Closed
