Delete me and disable me should have optional notification

Description
Optionally configure a notification in backend to send to moderators when a user deletes themselves or disables themselves.
A notification should also send to the user informing them of their action.
Include users current IP address in the notification as well as the date and time the action was taken.
- Status changed from Assigned to Resolved
- % Done changed from 0 to 100
- Status changed from Resolved to Closed
