Description

Simple reporting feature that displays a fontawesome flag icon. It then opens a dialog with a textarea to submit some more details. It then sends a simple moderator notification. Later a new plugin could be developed to handle the entire reporting process (probably a part of CB Notifications would make sense).

This should be only accessible to logged in users and have a 15min reporting delay so it can be spammed.

This should be provided by CB Notifications plugin.

For now just do simple reporting. If a user reports a status or comment it sends an email to moderators with a direct link to the status or comment and auto-hides it for the user reporting. On click display a simple modal dialog to supply a reason.

Now that published state is implemented it should keep track of the number of times the activity was reported. Once it hits a certain threshold it should automatically unpublish until it has been reviewed. Easy way to do this is store that information in params like the modify date. Probably need a parameter to set the report limit before it's blocked (this would be global only and not per stream).
