Standalone comment styling broken

Description
When a comment stream is displayed as standalone (not as part of an activity entry for example) the styling on it is broken. Redo the styling for standalone comments display so it looks better.
Normal styling also seams broken when the avatar isn't very square. Comment footer should go inside the comment body so it aligns properly.

Fixed the normal styling. Still needs standalone styling improvements.
