Improve read table usage
Description
Currently read table is asset based, which doesn't make any since as it's just like the hidden usage. Change asset to Type and Item so we can do a faster matching of read state. For example if a notification read state needs to be checked the Type is "notification" and Item is the notification id.

This isn't necessary as we don't want to keep track of individual read state. We only need to keep track of when the primary asset was read. Remove this functionality entirely from Activity and Comments APIs as it should be exclusively for Notifications and Conversations API. As for the button outputs for Activity and Comments just display the regular total count.
