Improve blocking to output generic avatar, canvas, and name

Description
Currently their output is suppressed entirely. This causes blank spaces in designs throughout the site. Instead it should output the empty avatar and canvas fields. Name can probably be the users formatname field value except not linked.
Basically just add a default field override for canvas, avatar, and formatname fields.
Display override parameter also needs to always be accessible since the display maybe blocked by user blocking and not by field privacy controls.

The following fields now have default display overrides.
canvas
avatar
formatname
firstname
middlename
lastname
name
Name fields will show the users actual name field, but will not link to their profile. The canvas and avatar fields will show empty canvas and avatar images, which will output defaults.
