Replace nested reply streams with a toggle
Description
Currently nested replies are just another full comment stream. Replies should just be a depth control for the existing stream. So instead of having to create multiple comment streams when enable replies simply toggle replies on/off and then a parameter to control reply depth. So for example you can enable replies and have a depth of 3 allowing to comments at a depth of 3. This means technically unlimited depth is also supported.

Additionally add a "parent" column to the comments table. Store the parent comment id in this. Replies should then have their "asset" set to same as parent and should not be using their own new asset. See #8814
