Replace thumbnail rendering with background images

Description
These are outputting images directly. Replace these with background images that are centered and non-repeating with 100% width/height. At the very least apply this to video thumbnails.

- Status changed from Assigned to Resolved
- % Done changed from 0 to 100
Thumbnails are now always guaranteed to be an image (can no longer output an iframe or video). Type specific fallbacks are provided for missing thumbnails.
