CB Paid Subscriptions

Replace setup_intent usage for subscriptions entirely. Currently the workflow is as follows.

1. Create customer
2. Create setup intent
3. Confirm and bill setup intent
4. Create subscription

This can be replaced with the following.

1. Create customer
2. Create subscription
3. Confirm and bill subscription payment intent
