CB Paid Subscriptions

When "Applicable to enabled/blocked users" is set to "only enabled users" emails are still being sent to unapproved users. Likely due to their accounts being enabled, but not approved. There maybe should be additional parameters to toggle sending to unapproved/approved and unconfirmed/confirmed.
