Replace the old cbtooltip modal behavior entirely with dialog elements.

The goal is to completely remove the need for cbtooltip. To minimize the amount of jQuery still in use as much as possible to improve rendering performance.

Modal window contents will still be hydrated on open though so that behavior will all remain the same.
