CB Paid Subscriptions

The parameter to toggle on billing address provided at time of payment is hidden and forced off. The only way to pass AVS is through the invoice address, but if the install doesn't use/need invoice addressing this makes it impossible to pass the billing address with a credit card payment. Change this parameter so it can be toggled within the gateway.
