Backend: Select "upload a new image" or "choose from gallery", then select "Approve image" does not hide upload/gallery

Description

Things appear to work if image is update through Profile Update method (backend shows 2 images and is functional - only small issue here is that if when you have 2 images you select 'Upload New Image' in backend you get the browse button to upload a new image. If you then select in the drop down the Approve image, the upload/gallery div is still showing)

If you try and use the Upload Image method from frontend menu, then:
#1 A message is properly displayed that image is pending approval
#2 The old avatar remains but it should have been replaced by the pending approval avatar
#3 The item does not appear pending in frontend or backend
#4 Old avatar is still present

update: bug was there in CB 1.2.2 also!!!

That one is fixed in commit r1068 .

I am not able to reproduce the 2nd main issue: tried old and new way of uploading image: works fine.

second issue appears to be a non-issue.

it was caused by one of my development plugins (profile update logger).
