Things appear to work if image is update through Profile Update method (backend shows 2 images and is functional - only small issue here is that if when you have 2 images you select 'Upload New Image' in backend you get the browse button to upload a new image. If you then select in the drop down the Approve image the Browse button is still showing)

That one is fixed in commit r1068 .

I am not able to reproduce the 2nd main issue: tried old and new way of uploading image: works fine.

second issue appears to be a non-issue.

it was caused by one of my development plugins (profile update logger).

Ok, Updating subject:

From: Old Image upload approval process broken + small ui bug in backend
To: Backend: Select "upload a new image" or "choose from gallery", then select "Approve image" does not hide upload/gallery

and closing as fixed.
