Image approval not functional for image fields
Description
Image fields (not the Avatar field) do not appear to respect the image approval configuration as configured in backend. Alternatively if image approval isn't supposed to work on image fields then the approved database column should be removed to avoid confusion and avoid wasted column space.

This is actually a new feature and not a bug. Would require new parameter to configuration and also be disabled by default to keep backwards compatibility.
