closedyesno in repeat causes no value to be selected on new row
Description
When adding a new row with a yesno usage it doesn't properly reset the yesno value resulting in nothing selected, but it should select the no. This will probably need a special case for yesno usage only.

This appears to be due to duplicate input names during the cloning process. It seams completely reliable when cloning the last row. So the cloning behavior may need to be adjusted to always clone and insert after the last row instead of the row being cloned.

Redo the repeat display so that there's no longer a + on each row. Always clone the last row. Move the + button below the last row and change to display "+ Add Row".
